Summary


The UT Limited (Community) Supports Waiver provides support for individuals with disabilities in Utah. It has no age restrictions, targeting those who demonstrate functional impairments in daily activities. Eligible individuals must have conditions that onset before age 18 for intellectual disabilities or before age 22 for related conditions. The waiver offers a variety of services including behavioral consultation, community transitions, personal assistance, and supported employment, promoting the ability to live safely and independently in the community while meeting Medicaid financial eligibility requirements.

Eligibility Requirements


  1. Must demonstrate functional impairment in 3 or more areas of major life activity.
  2. Onset of condition must occur before age 18 for intellectual disabilities.
  3. Onset of condition must occur before age 22 for other related conditions.
  4. Primary condition must not be attributable to a mental illness.
  5. Meet level of care criteria for admission to an intermediate care facility for people with intellectual disabilities (ICF/ID). ICF/ID is equivalent to ICF/MR as described under Federal law.
  6. Meet financial eligibility requirements for Medicaid.
  7. There are NO age restrictions for this waiver.
  8. Must be able to live safely in the community.
