Summary


The MN Elderly Waiver provides essential services to seniors aged 65 and older who require nursing home-level care but prefer to remain in their homes or community settings. Funded under the 1915(c) authority, this waiver allows eligible individuals to access a variety of services, including adult day services, case management, and home health services, to enhance their quality of life and maintain independence. The program aims to deliver these services at a lower cost than traditional nursing home care while ensuring that seniors receive the support they need in their daily lives.

Eligibility Requirements


  1. Be age 65 or older.
  2. Be eligible forĀ Medical Assistance.
  3. Need nursing home level of care as determined by a MnCHOICES assessment.
  4. Need services that Elderly Waiver can provide for less than the cost of care in a nursing home.
