Summary


The IL Persons Who are Elderly Waiver provides home and community-based services to eligible individuals aged 60 and older in Illinois. It aims to support those at risk of nursing facility placement and who can be safely maintained at home. Services include in-home assistance, adult day services, emergency home response, and automated medication dispensing. This waiver ensures that elderly residents receive necessary care while promoting independence and enhancing their quality of life within their communities.

Eligibility Requirements


  1. U. S. citizen or legal alien.
  2. Resident of the State of Illinois.
  3. Age 60 or older.
  4. Medicaid eligible.
  5. Be at risk of nursing facility placement as measured by the Determination of Need (DON) assessment.
  6. Estimated cost to the State for home care is less than estimated cost for institutional care.
  7. Can be safely maintained in the home or community-based setting with the services provided in the plan of care.

Services


In Home Service (Homemaker)

Provides assistance with household tasks such as cleaning, planning, and preparing meals, doing laundry, shopping and running errands. Homecare aidesĀ also assist customers with personal care tasks, such as dressing, bathing, grooming, and following special diets when the customer is unable to complete these tasks on their own.

Adult Day Service

Provides direct care and supervision in a community-based setting for the purpose of providing personal attention, and promoting social, physical and emotional well-being in a structured setting.
