Summary


The IA Brain Injury Waiver provides essential home and community-based services (HCBS) to eligible individuals with brain injuries in Iowa. Designed to support residents who require a level of care typically offered in institutional settings, this waiver enables access to various services, including case management, adult day care, and personal emergency response systems. Eligibility includes being a Medicaid recipient, having a qualifying brain injury diagnosis, and being at least one month old. The program aims to promote independence and enhance the quality of life for individuals with brain injuries while allowing them to remain in their communities.

Eligibility Requirements


  1. Be an Iowa resident and a United States citizen or a person of foreign birth with legal entry into the United States.
  2. Have a brain injury diagnosis included in a definitive list identified in 441 Iowa Administrative Code 83.81(249A). The Iowa Medicaid Enterprise, Medical Services Unit will confirm the diagnosis.
  3. Be eligible for Medicaid (Title XIX). Members may be Medicaid-eligible before accessing waiver services or become eligible through the waiver program application process. Additional Medicaid access opportunities may be available through the waiver program, even for those previously deemed ineligible.
  4. Be at least one month old.
  5. Be determined by the Iowa Medicaid Enterprise, Medical Services Unit to need a level of care that includes one of the following:
